Samaritan Internal Medicine-Corvallis (SIM-C) is a tier 3 medical home and residency clinic that offers patient-centered care. SIM-C recently joined forces with Albany Internal Medicine to create a larger internal medicine group that can share resources. The clinic maintains a focus on medical, social, behavior, and needed supportive therapies/services.
The Corvallis site offers physicians and nurse practitioners with integrated anticoagulation, clinical psychologist and a licensed clinical social worker. The clinic has two RN care coordinators that assist with individual plans and population-based engagement. It offers frequent classes such as diabetes class, chronic pain group, putting health first and smoking cessation.
SIM-C is entering its third year of Oregon’s Comprehensive Primary Care Initiative and is in its first year piloting the Alternative Payment Methodology Program with InterCommunity Network CCO (IHN-CCO). These programs have allowed the clinic to re-evaluate how it can take care of patients and identify ways to improve the way services are delivered.
