Make PCP Changes Online through Provider Connect

A new online tool is available immediately for primary care providers who have patients that are members of InterCommunity Health Network Coordinated Care Organization (IHN-CCO) and Samaritan Advantage.

PCP Assignment — accessible through Provider Connect — makes it easier for providers to assign patients to other primary care providers. Providers may assign patients to their own practice or clinic, remove them if they are no longer eligible, or list them as deceased. It is the provider’s responsibility to ensure that they have their patients’ permission before reassigning them to another PCP. 

Additionally, providers will no longer have to wait for a monthly report from Health Plans, as PCP Assignment will offer immediate access to daily reports, updated in real-time. 

PCP Assignment conveniently:

• Eliminates calls to Customer Service to request PCP changes
• Provides efficient and timely reporting
• Creates and mails new ID cards to members’ address on file
